November 2021 wurde das Innovationsfondsprojekt OPTINOFA mit dem 1. Platz in der Posterpr\u00e4sentation ausgezeichnet.<\/p>\n<p>Das Innovationsfondsprojekt OPTINOFA, unter der Verbundkoordination von Prof. Dr. Sabine Blaschke, Leiterin der Zentralen Notaufnahme an der Universit\u00e4tsmedizin G\u00f6ttingen (UMG), hat eine klare und differenzierte Steuerung von Notfallpatienten*innen in die verschiedenen Sektoren (ambulant versus station\u00e4r) der Notfallversorgung zum Ziel. \u00dcberf\u00fcllung, lange Wartezeiten und Qualit\u00e4tsrisiken in den Notaufnahmen sollen unter dem Einsatz eines Assistenzdienstes minimiert werden und damit der aktuellen Versorgungssituation in den Notaufnahmen von Kliniken entgegenzuwirken. Das Projekt nimmt sich aktuellen Forderungen der f\u00fchrenden deutschen Fachgesellschaften f\u00fcr Notfallmedizin, sowie der Gesundheitspolitik nach einer \u201eBedarfsgerechten Steuerung\u201c in der Notfallversorgung an.<\/p>\n<p>Der Posterpreis wurde f\u00fcr die Darstellung einer Interimsanalyse der projektbezogenen multizentrischen, kontrollierten Kohortenstudie zur strukturierten Ersteinsch\u00e4tzung in der Notaufnahme mittels intelligentem Assistenzdienst OPTINOFA verliehen.<\/p>\n<p>Bereits in den letzten zwei Jahren wurde das Projekt von den Deutschen Fachgesellschaften DGINA und DIVI mehrfach ausgezeichnet.<\/p>\n<p><strong>Das Projekt OPTINOFA und bisheriger Ergebnis<\/strong><\/p>\n<p>In Deutschland ist seit Beginn der letzten Dekade ein rasanter Anstieg der Fallzahlen in den Notaufnahmen der Krankenh\u00e4user zu verzeichnen. Dabei hat insbesondere der Anteil der ambulanten Notfallbehandlungen, die auch haus\u00e4rztlich versorgt werden k\u00f6nnten, \u00fcberproportional zugenommen. Wesentliche Ursachen f\u00fcr diese Entwicklung umfassen die demographische Entwicklung, patientenseitig steigende Qualit\u00e4tsanspr\u00fcche und Ver\u00e4nderungen der haus\u00e4rztlichen Versorgungsstrukturen in Verbindung mit einer r\u00e4umlichen und personellen Erweiterung der Dienstbezirke sowie Allokation von KV-Bereitschaftsdienstpraxen vorzugsweise an Krankenh\u00e4usern mit daraus resultierend l\u00e4ngeren Wegstrecken f\u00fcr Notfallpatienten. Die Folgen dieser Entwicklung sind rezidivierend auftretende Overcrowding-Szenarien in den Notaufnahmen, eine dadurch bedingte Erh\u00f6hung der Risiken der notfallmedizinischen Versorgung bei begrenzten Ressourcen sowie gesundheits\u00f6konomisch steigende Kosten der Notfallbehandlung.<\/p>\n<p>Im Innovationsfondsprojekt OPTINOFA wurde hierzu ein intelligentes, webbasiertes 5-stufiges Triage-Instrument zur strukturierten Ersteinsch\u00e4tzung von Behandlungsdringlichkeit und Notfallversorgungsstufe entwickelt und auf mobilen Endger\u00e4ten zur Verf\u00fcgung gestellt. Durch den Einsatz des OPTINOFA-Assistenzdienstes soll die differenzierte Steuerung der Notfallpatient*innen in den ambulanten und station\u00e4ren Sektor der Notfallversorgung erm\u00f6glicht und damit eine transsektorale Optimierung der Notfallversorgung erzielt werden.<\/p>\n<p>In einer kontrollierten, interventionellen, multizentrischen Studie mit elf teilnehmenden Modellkliniken, wurde das neue Triageverfahren eingesetzt und mit den international etablierten Triagesystemen (ESI und MTS) verglichen. F\u00fcr die Interimsanalyse wurden die Studiendaten aus acht Modellkliniken in Bezug auf das Ziel der neuen Versorgungsformen, sowie der Reduktion des Anteils ambulanter Notfallbehandlungen statistisch ausgewertet und der Interventionseffekt evaluiert.<\/p>\n<p>Die klinische Studie ist die erste kontrollierte, multizentrische Studie in Deutschland, welche die Anwendung eines neuen Triage-Systems zur strukturierten Ersteinsch\u00e4tzung der Behandlungsdringlichkeit und Versorgungsstufe (neue Versorgungsform) wissenschaftlich evaluiert. In der Interimsanalyse ergibt sich durch die Anwendung der OPTINOFA-Triage eine deutliche Reduktion des Anteils ambulanter Notfallbehandlungen in der Notaufnahme sowie konsekutiv ein Anstieg der Weiterleitung in den ambulanten Sektor.
